Medical and Spiritual therapies – Working Hand in Hand

Spiritual Health Questionnaire

About the Spiritual Health Questionnaire

Welcome to the CAMERS Questionnaire, a reflective self-awareness tool created to help individuals explore aspects of their spiritual well-being and inner life.

CAMERS is part of Heaven Foundation - Fondation CIEL’s broader educational and research work on spirituality, meaning, values, emotional-spiritual balance, and whole-person well-being. It is designed to invite thoughtful reflection, not to diagnose, treat, cure, or prevent any medical or mental health condition.

The CAMERS framework explores several areas we associate with spiritual reflection and personal growth, including:

  • Congruency: the relationship between personal values and daily life.
  • Mind or Thoughts: patterns of thought, reflection, and self-awareness.
  • Emotions: emotional openness, inner balance, and spiritual connection.
  • Response or Reaction: how a person responds to challenges, pressure, or conflict.
  • Speech: the role of words, expression, honesty, and communication.
  • Spiritual Orientation and Regulation: a person’s sense of meaning, connection, hope, and spiritual grounding.

Some earlier versions of CAMERS also included Attitude as a separate area. In the current framework, related elements have been integrated into the Mind or Thoughts and Emotions domains.

The Spirit Index is the companion summary framework connected to CAMERS. It is designed to help organize questionnaire responses into a simple reflective profile, showing areas of relative strength and areas that may benefit from further personal reflection, spiritual support, or growth practices. The Spirit Index is not a medical score, risk score, diagnosis, or measure of a person’s worth, faith, or spiritual maturity.

Our goal is to better understand how people experience spiritual well-being and to develop supportive educational resources, including spiritual guidance, reflection tools, coaching resources, and counseling-informed materials where appropriate.

Participation is voluntary. Your honest and thoughtful responses help us continue refining the CAMERS Questionnaire and the Spirit Index as reflective tools for spiritual growth and whole-person awareness.

The CAMERS Questionnaire and Spirit Index are offered for educational, spiritual, and research purposes only. They are not substitutes for medical care, mental health care, therapy, diagnosis, or advice from qualified professionals. If you have medical or mental health concerns, please seek support from an appropriate healthcare provider.
